Social Worker (HUD/VASH)

Responsible for providing case management from Veterans in the HUD-VASH program utilizing the Critical Time Intervention model Responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements Performs site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes Verifies the Veteran's status, eligibility for VA medical care, and eligibility of program participation Responsible for collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures Coordinates care to ensure comprehensive service delivery, linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies Complies with criminal conflict of interest laws and Executive Branch Standards of conduct while serving as the HUD-VASH Case Manager Actively participates in the HUD-VASH Program conference calls and trainings offered by local, VISN and VACO level HUD-VASH Responsible for following local policy and SOPs for the HUD-VASH Program to include requirements for CARF and Joint Commission accreditation Actively engages in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences Responsible for improving professional skills, participating, planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested Performs accountability functions such as documenting all contacts with Veterans/families in the medical charts and completing statistical and other reports in accordance with VA Medical Center and respective professional discipline service policies Responsible for working with VA Medical multi-disciplinary teams and community agencies to strive toward developing more effective and efficient ways of delivering healthcare services to HUD-VASH Program Veterans Independently conducts psychosocial assessments to determine needs of Veterans and family members Utilizes the Diagnostic & Statistical Manual of Mental Disorders, 5th Edition (DSM-V) as the basis for diagnostic impressions and conclusions Develops person-centered, culturally sensitive treatment plans in collaboration with the Veteran/family to address mental health needs of patients utilizing Mental Health Treatment Suite Participates in discharge planning with other disciplines and is responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate Provides a full range of social work services within community only accepted standards of social work practice to include individual, family and group counseling as appropriate, under the supervision of the HUD-VASH Coordinator Possesses the skills to offer psychosocial interventions and measures outcomes to interventions Provides consultation/education to Veterans and families on community resources, advance directives, organ donation and VA benefits Acts as a liaison between VA and community resources, to include marketing and public relation duties as needed Provides case management to Veterans and families, as needed, throughout the continuum of care Coordinates treatment and communicate with the care lines to facilitate continuity of care for Veterans Participates in committee assignments, as deemed by the service manager Assists Veterans and family members with coping and dealing with the loss and grief experiences in disability, terminal illness and death Documents work interventions and activities in Veteran's clinical record utilizing CPRS and ensure appropriate handling when transferring of Veteran care Complies with national and local performance mandates, including timely health care screening and completion of applicable clinical reminders for all patients Maintains familiarity and responsibility for all performance measures relating to the HUD-VASH Program Work Schedule:

Be a citizen of the United States. Education:
Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure:
Must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency:
Must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). Grade Determination:
GS-9 Social Worker Experience, Education and Licensure:
None beyond the basic requirements. Demonstrated Knowledge, Skills, and Abilities. At the GS-9 level, candidates must demonstrate all of the following KSAs:
(a) Ability to work with Veterans and family members from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds utilizing counseling skills. (b) Ability to assess the psychosocial functioning and needs of Veterans and their family members, and to formulate and implement a treatment plan, identifying the Veterans problems, strengths, weaknesses, coping skills, and assistance needed. (c) Ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. (d) Ability to establish and maintain effective working relationships and communicate with clients, staff, and representatives of community agencies. (e) Fundamental knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology. GS-11 Social Worker Specialized
Experience:
Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings. OR Education:
In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. (b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. (c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. (d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. (e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
