Social Worker (Outpatient)

As a member of the Interdisciplinary Treatment Teams, the Social Worker works independently using his/her educational training, clinical experience, VA policies, and JCAHO standards in administering direct services Performs specialized treatment of complex physical and mental illness for patient's followed by the Hematology/Oncology, Hepatology/Infectious Disease, and Transplant clinics Provides psychosocial services for all outpatient specialty clinics Provides supportive counseling and appropriate case management services to reduce the Veteran's stress and maximize psychosocial well-being throughout all phases of cancer diagnoses and treatment Independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs Possesses the knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups Independently provides clinical psychosocial and case management services to eligible Veterans and their family members to meet biological, psychological, social and environmental needs Services include educating veterans and their families regarding the resources available to them both at the VA and in their community while providing clinical treatment, advocacy, and coordination of referrals to appropriate VA or community service providers/agencies as needed Provides case management services to veterans with complex physical and mental health problems as needed, to maximize outcomes of medical intervention by addressing psychosocial needs Possesses specialized knowledge of issues in medical hematology/oncology, hepatology/ infectious disease and/or transplant, including various cancers, Hep C and infectious diseases, their treatments, issues faced by people with cancer Hep C and HIV, community resources and specialized benefits Evaluates prospective transplant Veteran patients by conducting a psychosocial assessment Participates with a multi-disciplinary team to educate the patient/family and to determine if they are an appropriate transplant candidate Serves as the social worker for transplant related needs Actively participates as a member of the interdisciplinary transplant referral team which meets weekly to review referrals and coordinate effective treatment plans Upon referral, assesses the patient/family's understanding of his/her diagnosis and proposed treatments in the Hematology/Oncology, Hepatology/ Infectious Disease and/or Transplant Clinic settings, among all other specialty clinics as well; provides intervention to help the veteran/family cope with the diagnosis Assesses treatment options when factoring in work, family and the veteran's other obligations. The incumbent also addresses the impact of their illness on life issues Provides education, assessment and intervention when related to the patient's VA, social security and disability benefits Assists with outpatient placements into residential care facilities, skilled nursing facilities and adult day healthcare programs as appropriate Coordinates and facilitates psycho-educational groups at the VAMC for veterans and caregivers Gathers feedback in the form of surveys from the weekly cancer care psycho-educational group to measure the effectiveness of this social work group Makes appropriate programmatic changes to maximize group participation and social work practice Provides individual, family and group counseling as appropriate and makes referrals for other mental health services as necessary Facilitates family discussions about illness, healthcare and life sustaining treatment decisions Assists Veterans and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness and death Provides consultation/education to Veterans and families on community resources, advance care planning, life sustaining treatment decisions and VA benefits, providing referrals and linkage to resources with specialized benefits Acts as a consultant to other team/staff members regarding patients served by the outpatient specialty clinics Serves as an active participant in the treatment planning process with other disciplines when appropriate and facilitates communication between the treatment team and the family as appropriate Independently identifies high-risk patients and provides case management services Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members Work Schedule:

Be a citizen of the United States. Education:
Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure:
Must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency:
Must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). Grade Determination:
GS-9 Social Worker Experience, Education and Licensure:
None beyond the basic requirements. Demonstrated Knowledge, Skills, and Abilities. At the GS-9 level, candidates must demonstrate all of the following KSAs:
(a) Ability to work with Veterans and family members from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds utilizing counseling skills. (b) Ability to assess the psychosocial functioning and needs of Veterans and their family members, and to formulate and implement a treatment plan, identifying the Veterans problems, strengths, weaknesses, coping skills, and assistance needed. (c) Ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. (d) Ability to establish and maintain effective working relationships and communicate with clients, staff, and representatives of community agencies. (e) Fundamental knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology. GS-11 Social Worker Specialized
Experience:
Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings. OR Education:
In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. (b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. (c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. (d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. (e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
