Senior Social Worker (PCMHI)
The Primary Care/Mental Health Integration Team (PC/MH Integration) Senior Social Worker operates within the Primary Care Clinic setting at the Fayetteville Health Care Center, Fayetteville, NC. Supports the primary care providers by providing continuity of care Provides mental health expertise and support to the Primary Care Clinic providers relative to Veteran patients experiencing primarily depression and/or problem drinking behaviors, although triage and initial psychosocial evaluation may occur related to other concerns such as thoughts of self-harm/suicide, anxiety disorders, adjustment disorders and grief/bereavement Provides direct services for the Veteran within the Primary Care Service Line that currently are new to the VAMC or who do not have a mental health provider Provides training for other members of a multidisciplinary treatment team in areas related to mental health assessment, treatment, and behavior management, life skills, and co-occurring disorders Independently conducts intake and needs assessment of Veterans that present to PACT for services related to a mood disorder Identifies symptoms leading to clinical diagnoses of a mental health disorder Provides all necessary assessment functions, e.g. interviewing, psychosocial histories, and mental health assessments to aid in the development of treatment plans and clinical assignments Performs case finding functions and outreach, identifying and screening patients for psychosocial needs Possesses the knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups Develops treatment plans with the Veteran that address the factors that contributed to mental health referral and other co-occurring factors within a recovery model framework Works with the Veteran to realize the Veteran's treatment goals, and reassesses and revises goals as indicated by the Veteran's engagement in treatment Provides Social Work support to the PC/MHI and the PACT Teams as a functional member of both teams, to include consultation and interventions Develops partnerships and knowledge of community agencies, especially those serving seriously mentally ill Veterans and providing community mental health to Veterans, with regular contact and communication Independently identifies high-risk patients and provides case management services Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members Possesses working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures; including acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology Coordinates contact with the facility Suicide Prevention Coordinator where clinically warranted for a veteran patient under his/her care Provides liaison services to other programs with the MHSL to ensure continuity of care and promote successful transition of Veterans from one program to another Facilitates referrals based upon Veterans needs and eligibility Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs Provides consultative services to mental health providers, as needed trying to access primary care services and follow-up for their Veterans Establishes and maintains effective therapeutic relationships with Veterans and their families Independently works with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills Operates from a psychosocial rehabilitation and recovery model of care in all Veteran patient and family interactions Provides appropriate clinical documentation for all contacts with or about Veterans Provides consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to treatment plan Responsible for the collection of statistical data and for reviewing the data to improve performance and delivery of mental health services to PACT Team and their Veterans culminating in reports as requested for leadership Independently conducts psychosocial assessments Utilizes the Diagnostic & Statistical Manual of Mental Disorders, 5th Edition (DSM-IV) as the basis for diagnostic impressions and conclusions Develops treatment plans in collaboration with the Veteran/family and with the interdisciplinary treatment team Possesses the skills to offer psychosocial interventions and measures outcomes to interventions using data to make clinical decisions Work Schedule:

Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work Licensure:
must be licensed or certified by a state to independently practice social work at the master's degree level English Language Proficiency:
Must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f) Grade Determination GS-12:
Must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment, and are considered experts in their specialized area of practice. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience in a specialty. Demonstrated Knowledge, Skills, and Abilities:

Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Licensure/Certification:
Must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination. The full performance level of this vacancy is GS-12. References:
