Senior Social Worker (Caregiver Support Clinical Assessor)

The Caregiver Support Clinical Assessor Senior Social Worker serves the Caregiver and those Veterans with severe and persistent medical and mental illness who are part of the Caregivers Support Program Conducts community assessments in the Veteran's Home that are currently enrolled in the Caregiver Support Program Performs health assessment on the Veterans and/or Caregivers of the enrolled Veterans Independently conducts psychosocial assessments in the Veteran's home and/or at the nearest VA location; utilizing the most recent addition of the Diagnostic and Statistical Manual of Mental Disorders (DSM V) as the basis for diagnostic impressions and conclusions Assures continuity of care by using a team approach and sharing responsibilities while providing medical and mental health referrals for the Caregivers and/or the Veterans Provides complex clinical services to clients on a regular and continuing basis, including suicide prevention and crisis intervention Achieves program goals through assessments, crisis/stress management, as well as, develops linkages to community resources and provide referrals Works with Veterans and Caregivers in the program who have met the criteria for admissions into the Caregiver Support Program Provides veterans and their family members with community outreach, counseling, therapy, treatment and direct services and referral to VA and community resources Employs a variety of psychological, rehabilitation, social work, counseling and mental health techniques including but not limited to individual, group, educational, substance abuse and readjustment counseling Responsible for an ongoing assessment progress and treatment Performs administrative functions such as actively participating in clinic meetings; maintains accurate records of his/her clinical interactions and collaborates with other team members and with professionals, paraprofessionals, volunteers, and others in VA facilities and community agencies to help veterans recognize and resolve their problems and needs Provides training experiences for graduate social work trainees; may make community visits needed to effectively perform his/her responsibilities and may participate in research and pilot studies Develops person-centered, culturally sensitive treatment plans in collaboration with the Veteran/family to address health needs of Veterans Complies with national and local performance mandates, including timely health care screening and completion of applicable clinical reminders for all Veterans Performs case finding functions and outreach, identifying and screening Veterans for psychosocial needs and medical needs Provides assessment of substance abuse issues and refers to appropriate treatment programs as indicated Participates in the admission/screening/assessment process by interviewing Veterans and/or other stakeholders and completing intake data collection with purpose of ensuring Caregiver Support Program Mental Health Assessment are in compliance Participates in or facilitates selected supportive groups related to the treatment of Veterans and Caregivers in the Caregiver Support Program, conducts individual counseling and other social work privileged functions independently or under supervision as professional license dictates Provides a full range of therapeutic interventions including psychosocial assessment, individual therapy, group therapy, and psycho-educational classes Acts as a resource and liaison person to public and private providers and agencies to assure continuity of care and appropriate referrals for Veterans and their stakeholders as levels of care change Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs Independently identifies high-risk Veterans and provides case management services as needed Provides crisis intervention services, seeking to address the cause, as well as, the presenting complaint; coordinates family conferences and serves as liaison to family members Coordinates contact with the facility Suicide Prevention Coordinator, or Suicide Prevention case manager where clinically warranted for a Veteran under his/her care Establishes and maintains effective therapeutic relationships with Veterans and their families Independently works with Veterans and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills Provides consultation services to other staff regarding the psychosocial needs of Veterans and the impact of psychosocial problems on health care and adherence to a treatment plan Work Schedule:

Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure:
must be licensed or certified by a state to independently practice social work at the master's degree level English Language Proficiency:
Must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). Grade Determination GS-12:
Must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment, and are considered experts in their specialized area of practice. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience in a specialty. In addition to the experience above, the candidate must demonstrate all of the following KSA's:
Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Licensure/Certification:
